GSSD Team Documentation Form
GSSD Team Documentation Form – This form is used to track all contacts made regarding a specific student including parent, school staff, professional service provider, and outside agency support. Please include an updated copy of this form in the Intensive Support package each year.
Authorization for Immediate Sub
Authorization for Immediate Sub EA Support Form – This form is completed by prinipals and authorized by SSCs or Superintendents for Emergency support. If approved, it is only valid until the next Intensive Support Intake Meeting
Intensive Support Request Form
GSSD Intensive Supports Request Form – This form is used to request support for a student with intensive needs. This form must be submitted to your SSC (along with the Ministry Impact form) one week prior to scheduled ISC meetings. Please see Student Services calendar for these dates. Your SSC will need to observe the student as well.
